While an internal investigation has been done of the death of a baby at the Diamond Regional Hospital, the Ministry of Health says an external investigation will also be undertaken. The Ministry yesterday expressed condolences to the family of the baby of Wendy Wharton, who died on October 12 and has set out the sequence of events before and after the death of the infant girl at the Diamond Diagnostic Hospital, East Bank Demerara. A release from the ministry said that immediately following the event, the hospital initiated an internal clinical investigation led by the attending medical officers as well as the midwifery and nursing teams. The review was conducted under the supervision of the hospital’s Quality and Patient Safety Department, in keeping with MoH’s established standards for the reporting and assessment of critical incidents.
