Washington State Health Department Takes Action Against Health Care Providers for Licensing Violations

The Washington State Department of Health has taken actions such as denials, charges, reinstatements, and probations against health care providers. Its Health Systems Quality Assurance Division oversees more than 80 health care professions and works with boards and commissions to enforce licensing standards, according to the Washington State Department of Health.

Bryan Fairbanks was denied orthotist and prosthetist credentials in Benton County for not disclosing a previous suspension in another state. In King County, Nicholas David Shlafer was granted an agency-affiliated counselor credential with probation. In Skagit County, Cecil Gordon Dodd received a substance use disorder professional credential but was placed on probation, as reported by the Washington State Department of Health.

The state has taken disciplinary action against health care professionals in several counties. In Clark County, massage therapist Jennifer Lee Allred was placed on probation for not following previous requirements and having incomplete patient records. In Island County, veterinarian Eric E. Anderson was put under oversight and fined for substandard care.
